Implants are typically placed:

  • Beneath the muscle (submuscular)
  • Partially under both (dual plane)

Choosing Your Implants

Mr Shoaib prefers the latest generation form-stable silicone gel implants for their safety, structure, and natural feel. These “cohesive gel” implants don’t leak and hold their shape even if damaged.

Implants vary by:

  • Shape: Round or anatomical (teardrop)
  • Surface: Smooth or textured
  • Filling: Silicone (most common) or saline
  • Projection & width: Matched to your body measurements

Pocket Placement: Where Implants Are Positioned

The implant must be placed in a “pocket” created within the chest area. The ideal placement depends on your anatomy, the type of implant, and the look you wish to achieve.

1. Submuscular (Under the Muscle)

  • Best for patients with minimal natural breast tissue
  • Helps reduce implant visibility and palpability
  • May lower the risk of visible implant edges
  • Slightly longer recovery
  • Risk: Double bubble effect if tissue slides over time

2. Subglandular (Over the Muscle)

  • Suitable if there’s already moderate breast tissue
  • Ideal for more enhanced or defined results
  • Shorter recovery period
  • May not conceal the top of the implant in very slim patients

3. Dual Plane (Partially Under the Muscle)

  • Combines the benefits of both placements
  • Good for women with low breast fold or minimal upper pole volume
  • Reduces risk of double bubble and offers a natural shape
  • Requires precise technique and inframammary incision

Incision Options

Implants are inserted through one of several incisions. Each has pros and cons that will be discussed in your consultation.

Inframammary Fold (IMF) Incision

  • Hidden in the crease beneath the breast
  • Well-concealed and preferred for larger implants
  • Offers excellent control during surgery

Mastopexy (Lift) + Augmentation Incisions

  • If combining with a breast lift, incisions may follow a lollipop or anchor pattern
  • Involves repositioning the nipple and removing excess skin
  • Custom plan tailored to the amount of lift required

Periareolar Incision

  • Half-circle incision around the lower areola
  • Heals well and is discreet
  • Only suitable if the areola is large enough to accommodate the implant

Axillary (Armpit) Incision

  • Leaves no scar on the breast
  • Best for small implants
  • Higher risk of bleeding or infection, slower healing
  • Less precise and not offered by Mr Shoaib

TUBA (Trans-Umbilical)

  • Scar hidden in the belly button
  • Only suitable for saline implants
  • Less precise and not offered by Mr Shoaib
